The Hospital Discharge Scheme has been operating since February 2007. The scheme assists in the reduction of people being readmitted to hospital and aims to promote a good quality of life and independence for all older people in Sunderland.

Do I qualify?

If your over 60.
Live in Sunderland.
Without a Care Plan.

It’s available to those returning home after a stay in hospital and either live alone or have no other support available. The service aims to assist people in the early days after discharge to settle back into home life.

What can we do for you?

shopping.
light housework.
Cook.
collecting prescriptions.
or simply just companionship.

Our FREE service is provided for upto 3 visits following discharge and at a time convenient to you and over a period of time that suits you, because we are all individual and may need different levels of support.

We make visits 5 days a week between the hours of 9 a.m. to 6 p.m.
