The Hospital Discharge Scheme is available to patients aged 65 and over who are returning home after a stay in hospital, and who either live alone or have no other support available. The service aims to assist people with discharge and help them settle back into home life. Help can be provided for the first two to three days following discharge with things such as shopping, light housework, collecting prescriptions, or simply just companionship.
